The YMCA Community Health Strategies team follows the Community Health Worker (CHW) Model. YMCA Community Health Workers serve as a trusted community advocate and frontline grassroots public health liaison who empower individual and community capacity by increasing health knowledge and self-sufficiency through outreach, community education, informal counseling, and social support with a culturally competent lens.
The Y hires, trains, and empowers lay leaders who are trusted members of and/or have a close understanding of the community served and identifies with the social, economic and health challenges of the community served. This relationship is key to ensuring equity for all aspects of the Community Health Strategies scope of work.

A COMPASS TO A BETTER YOU
The Health Navigation program (YHN) addresses health disparities and inequities in Broward and Miami-Dade County. Our team is comprised of Community Health Workers who assist residents in accessing clinical and community resources tailored to their needs. Participants are placed in one of three tiers based on their health needs and utilization of clinical services. Through connections to healthcare providers, mental health services, and community resources such as food distribution and housing assistance, the program aims to empower participants to better manage their overall health and well-being, all at no cost thanks to support from foundations, donors, and volunteers. COMMUNITY HEALTH
HEALTH EMPOWERMENT
Ready to gain the knowledge and tools to improve your health? Join us for any of our Health Empowerment workshops which include chronic disease and diabetes self-management, fall prevention, and healthy living strategies related to dealing with difficult emotions, healthy eating, safe exercise, pain management and more!
With sessions meeting once or twice a week for up to 6-8 weeks, you’ll gain valuable knowledge and tools for healthier living. Themes and schedules of workshops vary, ensuring there’s something for everyone. Participants often receive a certificate for completing each workshop.

NAVIGATE TO A SUCCESSFUL CAREER
The Employment Navigation Program will help you obtain meaningful training and career enhancement experiences for greater employment opportunities. You will work one-on-one with an Employment Navigation team member to acquire the resources and tools needed for your individual needs. LaBodega is a beacon of hope nestled within the vibrant tapestry of Miami’s Allapattah neighborhood. Funded by Florida Blue and in partnership with Feeding South Florida, Farm Share, and Food Rescue, LaBodega is our food pantry that provides essential nourishment and support to those facing food insecurity.
